Our Health Services Team is pleased to once again offer flu vaccinations to our middle and high school students. This year FluMist will be administered which is a vaccine that is sprayed into the nose to help protect against influenza.
If you are interested in having your child participate in the School Influenza Vaccination Program, please carefully read the enclosed vaccine information statement (VIS) regarding the Live, Intranasal Influenza Vaccine. Complete and return the Screening form and the School Influenza Vaccination Program consent form to your child’s homeroom teacher. Forms must be returned one week prior to the scheduled clinic at your child’s school. Vaccine will not be administered to any student without a signed parent consent form. After receiving the vaccination, your child will be handed a form documenting the administration of the seasonal flu vaccine for your records. Please share this information with your child’s Primary Care Provider.
